Protected Class Discrimination/Harassment and Retaliation
This form allows any person who has information about conduct that could be discrimination or harassment based on a protected characteristic to make a report or, if appropriate, file a complaint with the University’s Office of Civil Rights and Investigations (OCRI). Protected characteristics include race, color, national origin, religion, sex, pregnancy and related conditions, parenting status, disability, age, and military/veteran status. Individuals may also use this form to make a report or complaint of retaliation for engaging in a protected process. Protected processes include reporting a perceived wrongdoing, inequity, or violation of law or UI policy, filing a complaint alleging illegal or prohibited discrimination, participating in a grievance or appeals procedure, or participating in a dispute resolution through Human Resources or the Office of the Ombuds.
If your concerns pose an imminent risk or threat to the health or safety of any person in the university community, including yourself, you should immediately contact Campus Security at 208-885-SAFE (7233) and local law enforcement (911) before completing this form.
