    How to request health coverage tax documents

    Due to recent changes in federal requirements, the university will no longer automatically mail Form 1095-C to employees on university medical coverage. Form 1095-C, which confirms that you were covered under health insurance, is not required to be filed with your tax return.

    You may receive a copy of your 1095-C upon request. Please fill out a request form for the quickest processing. You may also send a request by mail to:
