General Forms

Completing and returning this form will allow financial aid staff to discuss financial aid matters with the individuals listed on the form. Persons granted information access are typically a spouse or parent.

Consent for Release

Students use this form to record a change to their enrollment plans (number of credits or semesters of enrollment).

Students use this form to record a change to their federal direct student loans (accept, increase or decrease).

Students use this form only if they are taking classes at another institution but would like to receive credit for the classes and financial aid from University of Idaho.

Short-term loans are designed to bridge an emergency shortfall between expected resources and payments that may fall due before a resource is available. Students use this form to apply for an Emergency Short Term Loan.

Cooperative Education is an educational model in which students alternate semesters of academic study with terms of full-time (paid) employment in positions related to their academic and/or career goals. This form is used to determine if a student wishes to utilize financial aid for the co-op semester or not. 

Appeal Forms

Students who have been placed on financial aid suspension will need to complete this reinstatement form. Once completed by the student, the form will be forwarded to the appropriate college contact for initial review before being sent to Student Financial Aid Services for final review.

Dependent students and their parent can use this form to appeal an extenuating circumstance which impacts their ability to borrow or repay a Parent PLUS loan.

Dependent students and their parent(s) may use this form to appeal FAFSA information based on changes to your family’s income, and/or excessive out-of-pocket medical expenses.

Students use this form to appeal FAFSA information based on changes to your income, excessive out-of-pocket medical expenses.

Dependent students and their parents may use this form to report a separation, divorce or widowed status that has occurred since the FAFSA was filed, a marital status that was mistakenly reported on the FAFSA, or to clarify marital status and income information if requested from Student Financial Aid Services.

Students use this form to report a recent marriage, separation, or divorce/widow status that occurred since the FAFSA was filed, a marital status that was mistakenly reported on the FAFSA, or if requested from Student Financial Aid Services as clarification for income information.
